[Job description] Care manager at the Regional Comprehensive Support Center [PA]
We mainly entrust the creation of preventive care plans for people requiring support. (Suggestion and coordination of commuting/visiting services, welfare equipment, etc.)

The person in charge is the user who can communicate. Visits are generally once every three months.

Adjust the number of assignments according to the number of working days.

Although I work at the Regional Comprehensive Support Center, my main job is to create care plans.
While supporting senior care managers, you can make use of your past experience to play an active role!

A total of 7 people are active, including 2 chief care managers, 2 public health nurses, 2 social workers, and 1 office worker!
