[Job Description] I would like to work as a "consultation desk" coordinating services such as welfare and medical care so that the elderly in the area I am in charge can live with peace of mind in a familiar area.
In addition, we will also be in charge of the following tasks, including acting as an application for certification for long-term care (required support).

・Preparation of long-term care preventive service plan (care plan) and coordination of services
・Support for mutual support activities in the community (collaboration with volunteers, organizations, etc.)
・Collaboration with related organizations such as care managers and medical institutions, and construction of a familiar support system
・Support for the protection of the rights of the elderly
